The COVID-19 pandemic created a spike in fraudulent claims filed with the Oklahoma Employment Security Commission (OESC). If you or your business has received a fraudulent claim from the OESC, the Oklahoma Attorney General’s Office is helping digitize the reporting process. In partnership with the OESC, the Attorney General is directing individuals and businesses who receive correspondence related to unemployment claims, that they did not file, to complete the form located here.
This form is for both individuals and businesses that have received a fraudulent claim. You must complete the form in order for the claim to be stopped.
If you are unable to complete all fields on the form, be sure to include all known information. Your claim will still be logged and move through the process.

- Save the document you received in the mail to your computer (There is a section on the online form to upload and attach the document; there is also a section to manually fill in the information if you do not have a way to upload the document electronically)
- Complete the online form and submit
